2. HOW THE
CONTEST WORKS: A) At the conclusion of each of the Events listed above, the
Sensodyne Event Team Representatives will review each of the videos submitted
at that Event and select the top five (5) videos according to the following
criteria: (a) 25% - Sincerity and persuasiveness
as a Sensodyne user; (b) 25% - usefulness of information conveyed as to causes
of teeth sensitivity; (c) 25% - camera presence; (d) 25% - effectiveness in
communicating and engaging audience in his/her personal Sensodyne story. In case of a tie, the tied videos will be
re-judged on criteria (d) and the individual scoring the highest on criteria
(d) will be selected. B) After the Sensodyne Event Team
Representatives have selected the top (5) Semi-Finalist videos, those five (5) videos
will be posted at www.SensodyneSearch site for fourteen
(14) days. During those fourteen (14) days, consumers can visit the site and
vote for their one (1) favorite Semi-Finalist video for that Event. In case of a tie, a random drawing will be
held to determine that Event's Finalist video.
Limit: only one vote per person or e-mail address per Event. A total of 15
Finalist candidates will be selected in this manner by consumer vote. C) The Sensodyne Brand Team, may at its discretion, select any video from each Event
listed above to be an additional Finalist.
D) The Sensodyne Brand Team will also judge the videos created, uploaded
and submitted prior to
3. PRIZE: The one (1) Finalist who is selected as "Sensodyne's Most Sensitive” person ("Winner") shall
win Two Hundred Fifty Dollars ($250) and receive the opportunity of appearing
in a future Sensodyne national advertising campaign. The Winner will be
notified by phone/e-mail/ or postal mail on or about

Eligibility: Contest is open to legal residents of the 50 United States
and District of Columbia, 18 years and older as of date of entry (residents of
Alabama and Nebraska must be 19 years and older as of date of entry) except persons who are professional models or
who are officers, directors, or
employees (and members of household or their immediate family, i.e.
spouse, parent, child, or sibling) of Sponsor; its affiliates, subsidiaries,
advertising and promotion agencies, including Mercury Promotions &
Fulfillment; Grey Advertising; and other individuals/entities associated with
this promotion are not eligible to enter or win. Winner must sign and return Affidavit of Eligibility/Compliance
& Publicity/Liability Release Agreement and any other documents as
requested by Sponsor within time period stated in notification letter or Prize
may be forfeited and awarded to an alternate potential Winner.

Internet: Sponsor
is not responsible for interrupted or unavailable network server or other connections,
miscommunications, failed telephone or computer transmissions or jumbled,
scrambled or misdirected entries or transmissions, or for phone, electrical,
network, computer hardware or software or program malfunctions, failures or
difficulties or for other errors, omissions, interruptions, or deletions of any
kind whether human, mechanical or electronic or for any damage to any person’s
computer related to participating in the Contest. Sponsor is not responsible for illegible, unintelligible, late, lost or
stolen entries or incorrect or inaccurate entry information, whether caused by
Internet users or by any of the equipment or programming associated with or
utilized in the Contest promotion or by any technical or human errors which may
occur in the processing of any entries in this Contest promotion. Persons found tampering with or abusing any
aspect of this Contest, as solely determined by Sponsor, will be disqualified
and may be subject to prosecution. If in
the judgment of Sponsor, the Contest is compromised by virus, bugs,
non-authorized human intervention or other causes beyond the control of
Sponsor, which corrupts the administration, security, fairness or proper play
of the Contest promotion, Sponsor reserves the right, in its sole discretion to
discontinue, suspend or terminate the Contest promotion. Any personally identifiable information
collected during your participation in this Contest promotion will be collected
by Sponsor and used by Sponsor and
its agents solely for purposes of the proper administration and fulfillment of
the Contest promotion as described in these Official Rules and will not be
shared with any third party except as stated in Sponsor's Privacy Policy, posted online at www.glaxosmithkline com and as allowed
by law. CAUTION: ANY ATTEMPT TO DELIBERATELY DAMAGE ANY WEBSITE OR
UNDERMINE THE LEGITIMATE OPERATION OF THE CONTEST IS A VIOLATION OF CRIMINAL
AND CIVIL LAWS. SHOULD SUCH AN ATTEMPT BE MADE, THE SPONSOR RESERVES THE RIGHT
TO SEEK DAMAGES OR OTHER REMEDIES FROM ANY SUCH PERSON(S) RESPONSIBLE FOR THE
ATTEMPT TO THE FULLEST EXTENT PERMITTED BY LAW.
In the event of a dispute as to the identity of a winner based on an
e-mail address, the winning entry will be declared made by the authorized
account holder of the e-mail address submitted at time of entry.
"Authorized account holder" is defined as the natural person who is
assigned to an e-mail address by an Internet access provider, on-line service
provider or other organization (e.g., business, educational, institution, etc.)
that is responsible for assigning e-mail addresses for the domain associated
with the submitted e-mail address.

Videos: By
submitting video whether through Rule 1(A) or (B) above, entrant represents
that he/she is the person shown in video, that he/she has the right to so
submit the video; that he/she waives all rights of privacy and any other
intellectual property rights in the video; and that Sponsor, et al. have the
right to use entrant's name and video (or portions of such video) for
balloting, voting, and such other promotional/advertising purposes in
connection with the Contest as they so determine without additional
compensation or consultation, except where prohibited by law. Videos created,
uploaded and submitted as stated in Rule 1(B) above must not include third
persons or the intellectual property of others (e.g. a logo or a registered
trademark. By appearing and/or submitting a video, videos submitted become the
sole property of and will not be returned.
7.
General: Void
