"SENSODYNE DENTAL SPA TRIPS 2010"


  1. NO PURCHASE NECESSARY. A copy of these official rules are available by sending a self-addressed, stamped envelope to "Sensodyne Dental Spa Trip 2010", c/o Dicom Promotions, 1650 Des Peres Road, St. Louis, MO 63131. One entry per household. Entries must be received by 10/19/2010. Dicom Promotions, GlaxoSmithKline and Mosaic Sales Solutions U.S. Operating Co., LLC. will not be responsible for lost, misdirected, illegible postage due, late entries, or failure in the delivery of any item sent via electronic transmission.
  2. HOW TO PLAY. No purchase necessary. Register by completing sweepstakes entry form found at Sensodyne.com.
  3. PRIZES: APPROXIMATE RETAIL VALUE PER PRIZE: $2,000-2,500 Eleven Grand Prize trips will be awarded. Ten (10) Prizes will be given to each of the radio programs participating market/retailer's…see attached exhibit A for market list and one (1) "national" prize for all entries
    Winners will have a choice of the following prizes:
    1. Trip for two to Ainsworth Hot Springs in British Columbia, Canada including roundtrip coach airfare for two to nearest major airline destination city and all transfers and 7 nights' deluxe hotel accommodations.

      2. Hollywood premiere of a potential Oscar winning movie includes roundtrip coach airfare for two to Los Angeles or New York, deluxe hotel for 3 nights and tickets to the premiere.
      3. Exotic Island includes roundtrip coach airfare for two to Bahamas nearest major airline city and all transfers and deluxe hotel for 5 nights.
      4. North Pole includes roundtrip coach airfare for two to nearest major airline city and all transfers, accommodations for 5 nights with sightseeing tour.
      5. Active volcano includes roundtrip coach airfare for two to Seattle, WA and all transfers, deluxe hotel for 5 nights with tours of both Mt. Mt. St. Helen and Mt. Rainier volcanoes.
      Cash Option of $1,500 is available per prize winner.

      All prize taxes, if any, are the responsibility of the winner. Limit one prize per person, household, group or organization. Winner will be determined by an independent, random drawing. All unclaimed prizes are forfeited and will not be awarded in a second chance judging. Winners will be notified by certified mail no later than 11/17/2010. Odds of winning depend on number of entries received.
  4. CONDITIONS OF PARTICIPATION. By participating in the sweepstakes, you accept and agree to these rules and the decisions of the judges, which shall be final and legally binding. You assume all risk of loss, damage, destruction, delay, or misdirection of game materials submitted for verification. All submissions become the property of GlaxoSmithKline and Dicom Promotions, Inc. and will not be returned. Winners may be required to sign an affidavit of eligibility and release and agree (except where prohibited by law) to the use of the winner's name, photograph and likeness for advertising or promotion and grant to GlaxoSmithKline and Dicom Promotions, Inc. and its affiliates any and all rights to said use without further compensation. Winners hereby release GlaxoSmithKline, Dicom Promotions, Inc., Mosaic Sales Solutions U.S. Operating Co., LLC. and, affiliated companies and their respective agents of all liability with respect to prizes. GlaxoSmithKline, Dicom Promotions, Inc, Mosaic Sales Solutions U.S. Operating Co., LLC. and affiliated companies are not responsible for printing, distribution or production errors and GlaxoSmithKline, Dicom Promotions, Inc. and or its affiliate may rescind any promotion found to contain such an error without liability at its sole discretion. As a condition of entering this sweepstakes, you agree that: (1) any and all disputes, claims and causes of action arising out of or connected with this contest, or any prizes awarded shall be resolved individually, without resort to any form of class action; (2) any and all claims, judgments and awards shall be limited to actual out-of-pocket costs incurred, including costs associated with entering this sweepstakes but in no event attorney's fees; and (3) under no circumstances will you be permitted to obtain awards for and you hereby waive all rights to claim punitive, incidental and consequential damages and any other damages, other than for actual out-of-pocket expenses, and any and all rights to have damages multiplied or otherwise increased. All federal, state and local laws apply. Contest void where prohibited, taxed or restricted. If there is any suspected evidence of tampering or technological corruption or if any portion of the promotion is compromised by virus, bugs, worms or unauthorized human intervention or any other causes beyond the GlaxoSmithKline, Dicom Promotion and Mosaic Sales Solutions U.S. Operating Co., LLC.'s control which, in the GlaxoSmithKline, Dicom Promotion and Mosaic Sales Solutions U.S. Operating Co., LLC. 's sole opinion, corrupts or impairs the administration, security, fairness, or proper play of the Sweepstakes, GlaxoSmithKline and Dicom Promotions reserves the right to terminate or suspend the Sweepstakes and select an alternative winner from entries received up to the time of such termination or suspension. GlaxoSmithKline and Dicom Promotions further reserve the right to disqualify any entrant, who, in its sole judgment, is believed by GlaxoSmithKline and Dicom Promotions to be tampering with the promotion in any manner whatsoever. GlaxoSmithKline, Dicom Promotions and Mosaic Sales Solutions U.S. Operating Co., LLC. is not responsible for lost, interrupted, unavailable network, server or other connections, miscommunications, failed phone transmission, cable transmissions, technical failures, garbled or jumbled transmission and the like, internet/network/ISP/website access/availability, or other errors of any kind whatsoever. In the event of cancellation or delay of space shuttle launch caused by any reason whatsoever after winner has booked trip, no revisions to initial planned trip schedule dates will be allowed.
  5. ELIGIBILITY. Participants and winner(s) must be U.S. residents at least 18-years old as determined by the Company. Employees and members of their immediate households, of GlaxoSmithKline and its advertising or promotion agencies, participating retailers or any other companies engaged in the development, production distribution of game materials are not eligible.
  6. SWEEPSTAKES SCHEDULE. Contest begins 5/10/2010 and ends 10/19/2010. Drawing will be conducted by Dicom Promotions, St. Louis, MO. If winner cannot be located or does not respond within 21 days of notification, the prize may be forfeited. Trip must be taken by December 31, 2011 (some blackout dates apply).
  7. WINNER'S LIST. For the names of the winners, mail a self-addressed, stamped envelope to Sensodyne Dental Spa Trips 2010, c/o Dicom Promotions, 1650 Des Peres Road, St. Louis, MO 63101.
